Prescott High School (Gr. 9-12)
Please complete the following registration forms and return to the Prescott High School Guidance Office, 1220 St. Croix St.
If you are new to the district and need to enroll your high school age child, please call 715-262-5389
Prescott Middle School (Gr. 6-8)
Please complete the following registration formsand return to the Prescott Middle School Counselor Office, 125 N. Elm St.
If you are new to the district and need to enroll your middle school age child, please call 715-262-5054, beginning August 6, 2018.
Malone Elementary School (Gr. K-5)
If you are moving during the school year, we encourage you to come to the school to register your child as soon as possible, even before you move if that is feasible. We realize that adjustment to a new school during the school year is difficult, but the less your child’s school year is disrupted by absence, the better.
Age: Kindergarten students must be 5 years old on or before September 1st. A copy of the child’s proof of age must be supplied at the time of enrollment.
Immunization: All students in grades K-12 must provide an immunization record at the time of enrollment.
Custody: If any person other than a natural parent has custody of a child, proof of legal custody or guardianship must be provided before the child may be enrolled in a Prescott public school.
How to Enroll: Complete the Prescott Community 4K Program Registration materials. Registration packets will be available at all of the centers as well as Malone Elementary School. Please return all registration materials to Suzi Linder firstname.lastname@example.org at Malone Elementary School.
Our school office is open from 7:30 AM to 4:00 PM each school day. You may come and register your child at any time during those hours or you may contact our office at 715-262-5463. Many of the required documents will be obtained from the previous school.
Moving to the Area During the Summer
We strongly encourage you to come by the school at your earliest convenience to register your child. We are out of the office for the month of July and return the first full week of August. Office hours are 7:30 AM-4:00 PM, Monday-Friday. We would love an opportunity to meet you and your children and to get them registered at Malone Elementary.
Getting your child registered early will allow us to obtain their records from the previous school, giving us important information needed to make an effective class placement for your children. It will also ensure that your children’s teachers will be ready to welcome them at our orientation day held on the first day of school.

To provide every four-year-old in the community with the opportunity to receive a developmentally appropriate learning experience in a quality group setting.
Benefits of Prescott 4K
Families can participate in the Prescott 4K program at no charge, at any of the three locations. Centers that offer extended child care services beyond school district sponsored programming will have fees which will apply if you choose to enroll your child. All children will have equal access to quality enhanced learning opportunities.
Instruction provided by an experienced Wisconsin DPI licensed teacher.
Children must be four years old by September 1 and live within the Prescott School District.

Our curriculum implements the Wisconsin Model Early Leaning Standards. These standards are based on current research and developmentally appropriate practices.
“Second Step” curriculum is used to foster all children’s social/emotional development.
“Get Set for School” a curriculum that focuses on language, literacy and math is used in all 4K classrooms.
